医改新明星！医用耗材占比逆天了

医药网2月22日讯　安徽省天长市最近格外火，医改“天长模式”上了央视《新闻联播》，招来了新华社，也招来了《人民日报》，国家级媒体纷纷点赞。 　　实际上，归属滁州市管辖的天长市，早在去年4月就被国务院确立为全国县级公立医院综合改革4个示范县之一。与其并列的还有江苏省启东市、福建省尤溪县、青海省互助土族自治县。福建省三明市是城市公立医院改革的典范，而县级医改，全国都要学这四家。 　　今年初，国家卫计委曾发文，拟对40个医改真抓实干成效明显的区县市进行奖励，安徽省有两地上榜，一个是蚌埠市，另一个也是天长市。 　　天长医改最亮眼的特色是“医共体”，然而它其实也是安徽省内先行的“耗材零差率”试点地区，也更早一些就减少了大型设备检查费用，以及打破医生薪酬的“天花板”和实行了医保支付制度改革等。 　　关于天长医改的成效，有一组数据：住院次均费用减少10.2%，药占比降至28.7%，耗材占比降至9%，平均住院日从10.8天降至8.6天，医务性收入占到60%，医共体内就诊率达到92.6%。（来源：《人民日报》） 　　这组数据是颇为惊人的。 　　因为国务院医改办给出的公立医院控费考核目标是：力争到2017年公立医院药占比（不含中药饮片）总体降到30%左右，百元医疗收入（不含药品收入）中消耗的卫生材料降到20元以下，个人卫生支出占卫生总费用的比例控制在25%左右。 　　当别人还在为如何将耗材占比降到20%以内绞尽脑汁的时候，天长市在2016年就已经降到9%了，不仅提前，而且远远超标完成了任务！ 　　那么天长医改究竟如何炼成？赛柏蓝器械结合《人民日报》、新华社、《安徽经济报》等的报道整理如下： 　　医共体掌勺，大小医院一个碗里吃饭 　　2015年，天长市以市人民医院、中医院和民营天康医院为牵头单位，组成3个县级医疗服务共同体，串联起14个乡镇卫生院、163个卫生室以及2个社区服务中心。 　　牵头医院对医共体内的人、财、物统一管理，医师多点执业，大型医疗设备统一管理、共同使用。医保基金也以医共体为单位，实行按人头总额预付制，超支由县级医院承担，结余由县级医院、镇卫生院、村卫生室按6∶3∶1进行分配。在这种框架下，县乡村逐渐形成分工明晰、配合默契的利益共同体，大家齐心协力把患者留在市域内，做大“医保”这口锅。 　　华中科技大学医药卫生管理学院教授张亮认为，天长的特点是以医保支付方式的改变，促进医共体的发展。 　　天长市在全省率先探索实行城镇居民医保、新农合、城镇职工医疗保险“三保合一”，成立医疗保险管理中心，由市医管会直接管理，并实施按人头付费、按病种付费、DRGs付费等多种支付方式。实行药品零差率后，降低药品医保支付价15%，并调整医疗服务价格。 　　协同发展的医共体顶住了周边大医院的“虹吸”，患者开始逐渐回流，医保资金也开始结余。 　　 “加减乘除”改革法 　　要把大部分病人留在县里，不仅是医保，人事、财政、医院等各方面改革都要触动原有格局。天长市委书记金维加总结为“加减乘除”改革法。 　　加，有4个方面：增强中心医院龙头作用，组建医共体，构建农村分级诊疗模式，即县级人民医院+乡镇+村卫生室；增加医生职责，为患者的健康管理服务；增加医改成效，承担大健康管理职责；增加财政投入，县级公立医院财政投入由医改前每年500万元，增加到2016年的2800万元。 　　减，有3个方面：减去药品加成，突破以药养医；减少大型设备检查费用；减去不合理收费。 　　乘，即调动医院和医生的积极性，使医疗服务效果翻倍。实行院长年薪制，市人民医院、中医院的院长年收入税后超过35万，由财政直接支付。普通医护人员收入与服务质量、服务数量、医改综合考核指标挂钩。 　　除，即着眼于平均数。比如，人均可报销比例等，以此衡量改革成效。截至2016年10月底，县域内就诊率92.24%，药占比30%以下，检查费用占比10%。 　　天长还成立书记和市长担纲的“双组长制”医改领导小组，组建由市长任主任的公立医院管理委员会，行使政府办医职能，给医改“算法”提供强有力的组织保障。 　　医疗接轨，强基层 　　 “医院想留住患者，不是靠控费，而是靠服务能力和医疗质量。”天长县级医院一方面密切联系南京、上海多家大医院，将专家请进来，一方面又将年轻医生送到南京大医院学习，积极培养自己的人才梯队。天长县级医院还购入3.0T核磁共振等大型设备，引入精准放疗等高端技术，做到医疗硬件、软件都与大城市医院接轨。 　　天长市财政投入2800多万元，完成镇卫生院改造扩建、医疗设备更新、村卫生室标准化建设，对基层医疗机构高级专业技术人员政府津贴。建村医准入退出机制，选聘村医571名。 　　在医共体内，县医院送医下基层，基层医院往县医院送培。牵头医院在人才、技术、管理、设备设施和服务等方面，加强对基层医疗卫生机构的支持和扶持力度，形成了1+1+1的师徒关系（县级牵头医院+乡镇医院+村卫生室）。 　　比如中医院牵头下的医共体，实行倒三角分配，结余中更大的比重分配给镇村单位进行医疗设施建设。中医院就给镇卫生院添置了大量设备，开通了远程会诊服务。 　　在医疗水平“追平”临近的大医院后，“性价比”优势开始凸显。 <span style="color: rgb(62, 62, 62);
