北京3600家医院耗材、设备采购使用要改革！

医药网3月23日讯　3月22日下午，北京市人民政府正式发布《医药分开综合改革实施方案》（以下简称《方案》）。从4月8号起，所有在京公立医疗机构将实施新的政策。 　　3600多家医疗机构要改革 　　改革范围包括北京市内政府、事业单位及国有企业举办的公立医疗机构和解放军、武警部队在京医疗机构。 　　政府购买服务的社会办医疗机构、城乡基本医疗保险定点的社会办医疗机构，可自愿申请参与改革。 　　根据北京市区两级有关部门多次组织核实和确认，参与改革的医疗机构达3600多所。 　　此前，北京市自2012年起，在友谊医院、朝阳医院、同仁医院、天坛医院、积水潭医院5家市属三级医院，首批试点了医药分开。 　　2014年至2015年，延庆、密云两区的6家区属二级医院，相继加入医药分开试点。 　　从此前的少数医院试点，一次性迈进到今年的全市3600家医疗机构全部都要改革，北京的动静要大了。 　　多项重磅改革，事关医械行业 　　《方案》提出取消挂号费、设立医事服务费、加强成本和费用控制、完善分级诊疗制度、建立财政分类补偿机制、加大医保保障和支付方式改革力度等内容。 　　其中下面这些与医疗器械行业关联较大： 　　1、阳光采购+医保控费，降器械虚高价格 　　在《方案》提出的几大改革目标中，有一项是：通过医药产品阳光采购、医保控费等措施，降低药品、器械、耗材等的虚高价格和费用。 　　其中，针对医用耗材是要推进京津冀联合采购，进一步降低耗材价格。 　　去年，北京市与天津市、河北省一起组成了京津冀医用耗材联合采购联盟，而首批止血和防粘连两类耗材的跨区域联合采购工作，已经在今年初启动了。三地耗材联合采购平台的一期建设也已经完成了。 　　至于医保控费，《方案》提出，要加大医保支付方式改革，推行医保基金总额预算管理，以及按病种付费、按人头付费、DRGs付费、按床日付费改革，逐步减少按项目付费。 　　2、大型设备检查，185个项目降价 　　在医疗服务项目价格调整上，这次改革选择了5类435个项目予以规范。 　　其中，价格上调的有4类250项，包括：综合医疗服务类（床位、护理等）125项，占28.7%；中医医疗服务类（针灸、推拿等）96项，占22.1%；临床手术治疗类（阑尾切除术等）26项和临床物理治疗类（放疗等）3项，共占6.7%。 　　价格下降的有1类185项，主要为CT、核磁等大型设备检查项目价格，占比42.5%。具体来说： 　　头部CT从180元降低到135元； 　　核磁从850元降低到400-600元； 　　PET/CT从10000元降低到7000元。<img data-s="300,640" data-type="png" src="http://mmbiz.qpic.cn/mmbiz_png/dx3KibwtDhvd8eecV8V55UhiasCZtU90VX4vXXRNHddwtUNvVFgAnibIz5GaWicsxULvLwEp3kP1xmgYjgTWYKDeZg/0?wx_fmt=png" data-ratio="1.1470588235294117" data-w="578" />部分项目价格调整情况 　　官方解读称，2015年北京市医疗机构医疗服务项目收入中，检查、化验和卫生材料收入占到66%，而诊查、手术、治疗、护理等项目收入仅占34%，大型设备检查、化验检验等硬物耗项目价格偏高，而医务人员技术劳务等软服务项目价格偏低。 　　因此，改革“一升一降”，既改变医院过去主要通过卖药品、用耗材、大检查、多化验等渠道获取收益、保障运行的补偿机制，也调动医生提高医疗服务水平的积极性。 　　3、强化大型设备购置管理 　　针对大型医疗设备，《方案》还提出，要加强对大型设备购置的可行性论证，提高医疗设备的使用效益。 　　大型设备购置如何进行可行性论证？ 　　作为首批5家试点医院之一，北京天坛医院是这么干的： 　　在大型设备的前期论证环节，由申请科室领导负责，在充分调研、方案比较的基础上编写可行性报告。对于使用耗材的医疗设备预算立项时，必须按照耗材准入原则和流程进行充分论证，如果耗材不符合准入条件，设备申请就不得立项。在设备的安装、入库环节，医用设备实行三级联合验收制度，由物资管理部、医学工程处、使用科室共同签署“安装验收报告单”，作为采购部启动设备结算的依据。 　　从一个设备立项开始，到论证、采购，再到后面安装调试、使用以及绩效考核和维修保养，一直到设备报残的整个过程，天坛医院的精细化管理工作贯穿于医疗设备的全生命周期。 　　另外，北京市本月初印发的《关于促进卫生与健康事业改革发展的意见》也已经提出，要加强成本核算与控制，严禁公立医院举债建设、举债购置大型医用设备。 　　4、耗占比降到20%以下 　　《方案》提出，通过改革，到2017年底，以行政区为单位，公立医院药占比（不含中药饮片）力争降到30%左右，百元医疗收入（不含药品收入）中消耗的卫生材料降到20元以下；到2020年上述指标得到进一步优化，公立医院医疗费用增长稳定在合理水平。 　　5、控制高值耗材的不合理使用 　　针对高值医用耗材，《方案》提别提出，要采用信息化手段，加大对异常、高额医疗费用的预警和分析，控制高值医用耗材的不合理使用。 　　对高值耗材的控制使用，和对大型设备的购置控制一起，成为公立医院“加强成本和费用控制”部分的突出要求。 <span style="color: rgb(62, 62, 62);
