医药网讯　安徽省食药监管局等五部门日前联合印发《关于印发安徽省公立医疗机构医用耗材采购“两票制”实施意见（试行）的通知》，宣布自2017年12月1日起，全省二级以上公立医疗机构实施医用耗材采购“两票制”，并对生产、销售不符合标准的医用耗材等违法犯罪行为及医用耗材购销中的垄断行为和不正当竞争行为进行重点打击。“两票制”“价格谈判”“联合阳光采购”“加大反腐力度”等规范药品采购流通的体系性制度，在医用耗材供应链购销端加速落地。 　　信息流规范升级 　　沿袭“高值耗材、普通耗材和检验试剂”的路径，挂网集采的核心目标是为理顺物流、信息流、资金流，从而提高交易流程透明度，节约医疗机构的采购成本，降低耗材试剂的市场价格。 　　广东省药品交易中心副总经理余峰近日在广东省药交所年会上指出，医用耗材本身存在极强的特殊性，包括产品名称申报不统一、业内使用产品俗称而不使用产品实际资质名称、部分产品须根据患者情况制定、流通环节不规范等，很多企业仍出于自身考虑要求医院线下采购。“各级经销商的价格不透明，但企业不要简单认为交易数据平台采集不到，只要存在交易行为，价格信息全省都可以采集，线上线下没有区别，医疗机构必须上传交易数据。” 　　前段时间国家发改委官微发布的《党的十八大以来医药价格改革纪实》明确，进一步取消医用耗材加成政策，推动破除“以械补医”；11月中旬，国家发展与改革委员会印发《关于全面深化价格机制改革的意见》，提出在巩固取消药品加成成果的基础上，逐步取消医用耗材加成。 　　综合运用信息化、标准化技术，理顺医用耗材购销链条，显然有利于全国医用耗材市场的规范化管理及耗材价格的制定和监管，实现全国医用耗材集中采购时不同地区间耗材及其价格的统计分析和追踪评价。多部门也相继提出取消医用耗材加成、各地纷纷发布医用耗材“两票制”政策，政府打击医用耗材价格虚高的决心不可动摇。 　　但是，“我国医用耗材购销分类与编码应用还存在很多问题，缺乏完整的体系，编码表现形式各异，数字顺序码、数字英文混合编码、条形码、二维码等均存在；由于缺乏全国统一的耗材编码，导致耗材物流、信息流、资金流无法整合共享，无法实现医用耗材集中购销在不同地区间对耗材及其价格的统计、分析、追踪及评估。”国家卫计委卫生发展研究中心支付制度与医学编码研究中心副主任于丽华指出。 　　供应链深度重构 　　在中国医药产业转型背景之下，医药商业领域的行业集中度提升，把握机会顺势而为，对每一家医药商业公司至关重要。监管层面，政府一直鼓励大型商业流通企业通过兼并重组、上市融资、发行债券等方式加快扩张整合，做大做强。对标美国及其他国家市场，美国前三强市场占有率为96%，日本是74%，德国为60%～70%，英国是85%，而中国的前三强加起来集中度还不够高。 　　只有充分的市场占有才能将商业流通市场整体运作成本降到最低，将流通效率提到最高，将种种弊端控制到最少。医药营销管理专家王强对记者表示，器械耗材领域深层次的医药商业供应链架构重构已经开始，此轮商业公司整合并非横向跨省域的资源聚拢，而是垂直资源嫁接，直接与终端纵深接入。 　　毫无疑问，只有深入调研，统筹规划，挖清医用耗材阳光挂网采购和“两票制”业务要求，分析现状困难和实施目标，做好顶层设计，才能保证耗材购销制度落实的整体性、系统性、可持续性。 　　湖北省公共资源交易中心采购科科长陆喆强调：“须制订阶段性工作计划，分步实施，逐步将高值医用耗材全部挂网；已有地市级耗材采购平台的需求逐步与省级平台融合，实现全省价格、渠道统一的线上平台，购销行为上线联动、全程监控；采集医院采购、使用、结算等数据信息，为‘医疗、医药、医保’联动提供系统性支撑。” 　　对于器械耗材各级代理商来说，从全国代理商到省级、地市级、县级、小包商均会受到不同程度的购销合规冲击，影响程度大小取决于是否需要高开、能否解决高开以及是否掌握终端资源。“国家版‘两票制’中，全国总代不可视为生产企业，下游客户会纷纷绕过它而从厂家直接拿货和开票。由于没有直接掌握终端资源，代理商被市场替换的可能性加大。”王强表示。<p
