医院落实耗材两票制文件，对械企提要求

<img data-s="300,640" data-type="jpeg" src="http://mmbiz.qpic.cn/mmbiz_jpg/dx3KibwtDhvcZMQ28lpH0lFyXic8ia00eQicvTrDdRenkcuSOC0dxWPX4AnBqlljy0ibd3awkDObkl8OT1D4QKwDA9A/0?wx_fmt=jpeg" data-ratio="0.67" data-w="1000" /> 医药网1月16日讯　有关耗材两票制的实施，不但涉及上游生产企业、中游配送企业，也涉及到终端的医疗机构。 　　在不少省份纷纷推动耗材两票制背景下，我们来看同时推进药品与耗材两票制的陕西省，其中的一家三级医院是如何落实两票制的，其落实过程中，又会对上游提出怎样的要求？ 　　根据商医改办发〔2016〕12号“转发陕西省深化医药卫生体制改革领导小组办公室省卫计委关于深化药品耗材供应保障体系改革的通知及（陕医改办发〔2016〕8号）”文件精神，商洛市中心医院医用耗材及试剂采购从2017年1月1日开始试行“两票制”，具体要求如下： 　　一、配送企业销售至医院为两票。医用耗材及试剂配送企业在供应产品提供发票的同时须提交由生产企业提供的进货发票复印件，发票复印件须加盖配送企业的红章（生产企业发票复印件清单内容应包含配送企业配送的品种名称、规格、数量、单价、金额等）。 　　二、生产企业直销至医院者为一票。以下情况可视同生产企业： 　　（一）医用耗材生产企业或科工贸一体化的集团型企业设立的仅销售本企业（集团）耗材的全资或控股商业公司、境外耗材国内总代理（仅限1家）可视同生产企业。 　　（二）医用耗材上市许可持有人委托耗材生产企业或经营企业代为销售耗材，可视同生产企业。 　　可视同生产企业的以上情况需向医院提交相关证明文件。 　　三、对执行“两票制”的配送企业医院付款优先。 　　四、为保证医院工作正常开展，对不能实现“两票制”的品种，医院设立过渡期，过渡期限为2017年1月1日至2017年4月30日。过渡期内配送企业可采取以下方法办理“医用耗材/试剂配送企业变更申请”或“医用耗材/试剂配送不能执行两票制声明”： 　　（一）如果现配送企业与生产企业协商后变更配送企业可实行“两票制”，可向医院提交“医用耗材/试剂配送企业变更申请”（格式见附件1、附件2、附件3），医院批准后可变更为新的配送企业配送。 　　但在医院未批准前现配送企业必须保证医院供货。建议变更的新配送企业为目前与医院有配送业务的企业。心血管介入及骨科类耗材变更的新配送企业必须为陕西省阳光采购平台挂网企业。 　　（二）如果现配送企业无法与生产企业商定新的配送企业，应向医院提交“医用耗材/试剂配送不能执行两票制声明”（格式见附件四），以便医院决策。但在医院未通知停止供货前现配送企业必须保证医院供货。 　　五、2017年1月起，“两票制”发票、“附件1：医用耗材/试剂配送企业变更申请”、“附件2：配送企业档案资料（试剂）或附件3：配送企业档案资料（耗材）”、“附件4：医用耗材/试剂配送不能执行两票制声明”，必须交设备耗材采购小组审核后再按照正常手续办理走账手续。 　　六、医院根据两票制执行情况适时启动集中配送企业遴选工作。 　　七、未尽事宜以陕西省医改办及省卫计委相关规定及解释为准。 <p style="font-size: 16px;
