<p><img data-type="gif" src="http://mmbiz.qpic.cn/mmbiz_gif/dx3KibwtDhvc6YBndBzrYvfAVPCGNAlsqB9f7SjRnXkibcO3P2CSjz7TnOO8RAArOGOjobic1D5DCTCzzAoxv9jfw/0?wx_fmt=gif" data-ratio="0.1703125" data-w="640" style="line-height: 1.6;" /><br /></p><p> <span style="line-height: 1.6;">医疗器械飞行检查浪潮席卷开来。</span></p><p> </p><p> 2017年，国家药监总局已经组织展开了2批飞检，共涉及16医疗器械生产企业。</p><p> </p><p> 除此之外，地方药监局也已经行动起来了。截至目前，安徽、上海两省市已经有多家械企飞检中被发现问题，遭受停产、整改、甚至是被查封、立案调查的惩处。</p><p> </p><p><strong> 安徽省：1</strong><strong>家械企被查封、2</strong><strong>家限期整改</strong></p><p> </p><p> 据安徽省药监局消息，3月31日，安徽省药监局飞检组突然对安徽鸿基伟业医疗器械发展有限公司进行检查。</p><p> </p><p> 飞检组发现，该企业天然乳胶避孕套注册证已于2016年9月3日过期，但企业仍在昏暗隐蔽的车间里热火朝天地进行生产、包装。执法人员经核查后，现场共清点出441桶（每桶约10000只）未包装的避孕套。</p><p> </p><p> 安徽省药监局飞检组对441桶未包装的避孕套和生产加工设备进行了查封，并责令太和县药监局对企业立案查处，责成阜阳市药监局挂牌督办，以及将检查结果通报给正在对该企业进行产品注册的部门。</p><p> </p><p> 此次飞行检查，还发现：安徽众康药械有限公司存在物料<strong><span style="color: rgb(0, 153, 102);">采购</span></strong>生产记录不完善、检验设备及试剂不能满足检验需求、原材料与供应商未进行分类管理等9项缺陷。安徽大千生物工程有限公司存在分析天平等检验仪器记录不全、生产记录存在随意涂改、部分原料实行编码管理但未建立相应的控制程序等7项缺陷。</p><p> </p><p> 上述2家生产企业，被飞检组当即责令限期整改。</p><p> </p><p><strong> 上海市：6</strong><strong>家械企停产，其中2</strong><strong>家已立案</strong></p><p> </p><p> 据上海市药监局消息，近日，该局组织对部分医疗器械生产企业开展了飞行检查。</p><p> </p><p> 现场检查发现，有4家械企存在不符合医械<strong><span style="color: rgb(0, 153, 102);">GMP</span></strong>相关规定行为，生产质量管理体系存在严重缺陷，上海市药监局责成企业暂停生产进行整改，并评估产品安全风险，对有可能导致安全隐患的，按照《医疗器械召回管理办法》的规定召回相关产品。</p><p> </p><p> 这4家医疗器械生产企业包括：上海硅莱医疗器械有限公司、上海博览光电仪器有限公司、上海卫生材料厂有限公司、上海丰汇医学科技股份有限公司。</p><p> </p><p> 其中，上海硅莱医疗器械有限公司和上海博览光电仪器有限公司在检查中还被发现，存在涉嫌违反《医疗器械监督管理条例》及相关规章的行为，由企业所在区市场监管局立案调查，依法处理。</p><p> </p><p> 此外，上海骁博科技发展有限公司也存在不符合医械GMP规定行为，企业的《医疗器械生产企业许可证》也已过期。上海市药监局要求，企业在重新取得生产许可前不得从事医疗器械生产活动。</p><p> </p><p> 上海欧亮医疗器械有限公司在飞检中被发现处于停产状态。上海市药监局要求，企业在恢复生产前应当按照《医疗器械生产监督管理办法》的规定书面报告所在区市场监管局，经核查符合要求后方可恢复生产。</p><p> </p><p><strong> 以下为各自问题详情：</strong></p><p> </p><p> 1、上海硅莱医疗器械有限公司主要存在以下缺陷：主要零部件（变压器、无油空压机）与注册证中的产品配置及主要部件信息不一致，但企业未办理变更注册，也未提供相关验证资料；移动式牙科治疗机产品注册证中要求洁牙机、光固化机应具有有效医疗器械注册证，无油空压机应具有有效第一类医疗器械备案凭证/注册证，但企业不能提供无油空压机的医疗器械备案凭证/注册证，仓库中的洁牙机、光固化机产品无中文标识；医用无油空气压缩机采购合同中无供方签字盖章，未明确规格型号等采购信息；未对仓储区内不合格品进行标识；未查见变压器（检验日期：2017.1.9和2017.2.17）的采购发票；牙科治疗机生产流程图中有过程检验，但企业未提供过程检验规程；企业未按标识和可追溯性控制程序的规定对A类物料进行追溯记录；企业未按检验规程的要求提供进货和过程检验记录；企业未提供产品放行程序；企业未按不合格品控制程序的规定提供过滤调压阀等不合格品处理记录。</p><p> </p><p> 2、上海博览光电仪器有限公司主要存在以下缺陷：裂隙灯显微镜主要零部件（跷板开关）制造商与注册证限定的不一致，且企业未提供新供方的审核记录，未签订质量协议，也无法提供采购记录；部分调试检验记录单有检验数据但无产品编号和检验员签字、日期；部分无检验数据的产品出厂检验报告中检验结论为“合格”，并加盖主检和批准人员印章；产品出厂检验报告中的检验结果汇总无人员签字和日期；未能提供新入职和关键工序生产人员培训记录；操作台上放置水杯、食品等个人生活用品；检验室靠窗墙壁上大面积墙皮剥落和水渍；零部件仓库内部分零配件无标识，部分关键部件的卡、物数量不一致。</p><p> </p><p> 3、上海卫生材料厂有限公司主要存在以下缺陷：实施受托生产企业现场审计时，相关审计表和审核表内容均由受托方企业自行填写；原材料仓库墙体斑驳，地面积水蔓延至放有药用原材料的垫仓板下；成品仓库无温湿度控制设施，无法确保储存条件符合产品说明书的要求；成品仓库内待检的退货弹性创可贴存放在合格区内；未查见内包装用封口机；洁净区内紫外线<strong><span style="color: rgb(0, 153, 102);">消毒</span></strong>灯无法开启；消毒棉球车间内尚处在设备验证阶段、未能正式投入生产的全自动消毒棉球机设备标识为完好、运行；脱脂棉生产车间内查见已作废的受控文件《清场标准操作规程》；企业未对消毒棉球主要原材料供应商进行定期审核；质量协议中未明确医用脱脂棉、医用纱布的生产洁净级别和初始污染菌要求。</p><p> </p><p> 4、上海丰汇医学科技股份有限公司主要存在以下缺陷：成品库查见2017年3月生产的体外诊断试剂成品，但企业未提供2017年净化车间、电子天平、灌装机等设施设备使用记录以及消毒剂配制记录；洁具间水槽生锈，排水管为直排管路，无防倒灌装置；成品库中已过期钙（Ca）试剂盒未放不合格品区，部分在库产品货架数量与货位卡不一致；退货区查见全英文标准品无数量记录；退货产品放置在经销品库，且无货位卡记录；未按规定开展制水设备维修、保养；配制器具没有状态标识；企业未能提供内包装在洁净室内暂存时限的验证报告；内包装暂存间内大量内包装无任何标识，也未标示是否清洗及清洗日期；检验室标准品冰箱内的“腺苷脱氨酶试剂标准液”、ADA质控（低）均已过期。</p><p> </p><p> 5、上海骁博科技发展有限公司主要存在以下缺陷：菌种冰箱无温度记录，冰箱无冷冻室，达不到-20℃的菌种储存要求；企业内包装间封口机封口参数已变更，企业未对封口温度和速度参数范围进行确认；封口强度确认数据没有原始测量仪器编号及原始数据记录，确认不充分；部分规格粒型产品的理化检测原始记录检测结果超出检测标准要求，判定结果仍为“合格”；原材料三氯甲烷、甲醇等标签标示应储存于阴凉、通风、避光环境，现场未配备避光、通风装置和温湿度调节及监控设备；已清洗的托盘摆放在标有“未清洗”标示的区域；内包装间内查见周转箱、修剪产品用剪刀无清洁状态标识；现场未查见生产用电锯使用记录；生产工艺要求冻干机工作压力为110K，现场未查见关键工序“干燥”生产设备冻干机真空表校准记录；净化车间空调系统未开启的情况下，二更门口的压差计显示为负压，内包装室、细菌检测室、阳性间对外压差为5-10pa；未查见纯水机压差表和阳性室生物安全柜的校准标识和校准证书；现场查见已作废的《医用诱导骨基质制造作业指导书》无作废标识。该企业的《<strong><span style="color: rgb(0, 153, 102);">医疗器械</span></strong>生产<strong><span style="color: rgb(0, 153, 102);">企业</span></strong>许可证》已过期，企业在重新取得生产许可前不得从事医疗器械生产活动。</p><p> </p><p> 6、现场检查发现，上海欧亮医疗器械有限公司处于停产状态。在恢复生产前应当按照《医疗器械生产监督管理办法》的规定书面报告所在区市场监管局，经核查符合要求后方可恢复生产。</p><p><img data-s="300,640" data-type="jpeg" src="http://mmbiz.qpic.cn/mmbiz_jpg/dx3KibwtDhvfA5YzcziaXnGzFwP9dA7s16UIhKCDVibDKthtt7xe0esStqMzXscWmrsb8hQ9xK1mSavbDiaV6SxIew/0?wx_fmt=jpeg" data-ratio="1.1956521739130435" data-w="460" /></p><p><br /></p><p style="max-width: 100%;
