ABO Blood Group and the COVID-19 Susceptibility”</span></span><span>(ABO血型与COVID-19易感性的关系)未经同行评议的文章引起了广泛关注,该研究首次发现:A、B、O、AB血型与新冠肺炎易感性存在关联。那么神奇的血型是否可以帮你豁免此次新冠瘟疫,下面我们来一探究竟。</span></span></p></section></section></section></section></section><section><p><br /></p><p><span><strong><span>研究发现:</span></strong>A血型的人比非A血型的人获得COVID-19的风险要高得多,而O血型的人比非O血型的人感染COVID-19的风险要低得多。我们来看看对比探讨一下。</span></p><p><br /></p><p><span>文章通过对武汉、深圳三家医院经SARS-CoV-2检测证实的2173例COVID-19患者与武汉地区3694例正常人的血型分布进行比较。</span></p><section><br /></section><section><p><span><span>A、B、AB、O在武汉3694例正常人中分别为32.16%、24.90%、9.10%、33.84%,而在武汉金银潭医院1775例COVID-19患者中,A、B、AB、O分别为37.75%、26.42%、10.03%、25.80%。研究结果提示:COVID-19组患者A、O血型比例分别明显高于和低于正常人(<span>P均< 0.001</span>)。在武汉和深圳</span><span>另外两家医院的398名患者中也观察到类似的ABO分布模式,如表1。</span></span></p></section><p><br /></p><section><span><strong><span>表1</span></strong>:<br /></p><p><span>从数据和结论上看都很合理。因文章未经同行评审,也有质疑之声,有几个不确定:</span></p><p><br /></p><section data-tools="gulangu" data-id="97592"><section><section><section data-autoskip="1"><p><span>1、对照数据为之前对武汉市3694名正常人的血型调查,这是一个历史数据,这个数据能否真正代表新冠肺炎患者“所属”人群本来应该有的血型分布?<strong>不确定</strong>。</span></p><p><br /></p><p><span>2、本组研究的是流行病学数据,研究的数据量有限,扩展到更多的人群中,是否还能达到统计学上的显著区别,<strong>不确定</strong>。</span></p><p><br /></p><p><span>3、相关性问题,这个研究结论只能说明血型和新冠肺炎之间有相关性,但并不能肯定病毒就容易感染某种血型的人群,能否证明ABO血型是COVID-19敏感性差异的生物标志物,<strong>不确定</strong>。</span></p><p><br /></p><p><span>4、285名来自广东省深圳市第三人民医院的 COVID-19患者,A、B、AB、O型血比例分别为28.77%、29.12%、13.68%、28.42%,而对应的之前的深圳市23368名正常人群的A、B、AB、O型血比例分别为28.77%、25.14%、7.32%、38.77%,如果比较这两个数据,O型血比例的比例是降低了,但COVID-19患者中A型血的比例是否升高了?样本数有限<strong>不确定</strong>。</span></p></section></section><section /></p><section><p><span>感染风险估计中,O型血的感染风险显著降低(OR为0.627;95%CI为0.484-0.812)。A型血型与感染COVID-19的风险显著升高(OR为1.21;95%CI 1.02-1.43,P = 0.027),而AB型血型与感染COVID-19的风险更高(OR,1.48,95%CI 0.97-2.24),那可能AB型感染COVID-19的风险更高。<br /></span></p></section><p><br /></p><p><p><img src="image/20201014/dce1814e142320ef95318baf902dd50d_6.jpg" /></p></p><p><br /></p><p><span>目前的临床观察提示,年龄和性别是COVID-19易感性的两个危险因素。老年人和男性更容易感染和发展更严重的疾病。然而,到目前为止,还没有发现可以预测COVID-19易感性的生物学标记。</span></p><section><br /></section><p><span>Patrice等人发现,抗A抗体特异性地抑制了表达SARS-CoV蛋白的细胞对表达ACE2的细胞系的粘附,由于SARS-CoV-2和SARS在核酸序列和受体血管紧张素转换酶2 (ACE2)感染途径之间的相似性,低敏感性的血型O和高敏感性的A型血感染COVID-19的风险可能与血液里血型抗体的存在有关。</span></p><p><br /></p><p><span>如果与血型抗体有关,A型血更易感,A型血血清中有抗B而无抗A抗体,红细胞膜上有A抗原。O型血不易感染,O型血血清中既有抗A又有抗B抗体,红细胞膜上A、B抗原均无,这样看来可能与<span><strong>抗A抗体有关</strong></span>。那么问题来了,B型血血清中也有抗A抗体,那么B型血是不是也可能不易感呢?这样看来这些假设都需要直接的研究来证明。</span></p><p><br /></p><p><p><img src="image/20201014/64b222a6c23cf50ae3b8ff81995893f9_7.png" /></p></p><p><br /></p><p><span>至于文章在死亡病例中,高危血型A和低危血型O的分布模式也与之相似。与非O型血组相比,O型血死亡风险更低,OR为0.660(95%CI 0.479-0.911,P=0.014)。相反,与非A型血组相比,A型血组的死亡风险更高,OR为1.482(95%CI 1.113-1.972,P=0.008)。那么不同血型对病毒的清除是否有关呢,我们来看看不同的血型抗原如何形成。</span></p><p><br /></p><p><span>ABO基因是产生特异性糖基转移酶(glycosyltransferases)的遗传密码。使糖类连接到基础前身物质(basic cursorsubstance)上,H基因的作用与ABO抗原的形成有密切关系,A、B及H抗原全部都是从相同的前身物质形成的。此基础物质主要为糖蛋白的糖脂,其连接的糖所需的特异性转移酶是遗传基因所合成的。</span></p><p><br /></p><p><span>H物质的形成,首先需至少一个H基因的遗传,以连接遗传的ABO基因产生的其他糖类。所有具有H基因的个体,H物质是最先形成,然后连接其他糖类到H物质上,具体接何种糖类,则须看所遗传的ABO基因而定。A基因的遗传产生N-乙酰-D半乳糖转移酶,使N-乙酰-D半乳糖与H结构相连接。</span></p><section><br /></section><p><span>这种糖与A的特异性有关。A基因产生的糖基转移酶浓度比B基因者为高。实际上这使红细胞上的所有H抗原转化为A抗原。在A型成人红细胞上存在的A抗原,可多达81万~117万个。而B基因产生D半乳糖转移酶,使D半乳糖的糖与且物质相连接。</span></p><section><br /></section><p><span>B型成人红细胞上存在的B抗原位点有只有60万~83万个<sup>[7]</sup>较A型少,COVID-19感染是否<span><strong>与A抗原位点有关</strong></span>?O基因为无效基因,不会产生转移酶,所以不会在H结构上加糖。因此,O型人只有高浓度的H抗原,H抗原是否缺少<strong><span>A抗原位点有关</span></strong>?O型血对糖基化的免疫反应相对比较强,病毒的蛋白外壳上也有很多糖基化,是否O型血的人因为针对糖基化的免疫反应普遍比较强,因此有助于更快地清除病毒,而感染风险和死亡风险相对较低呢?这些都需要进一步的研究证实。</span></p><section><br /></section><section data-tools="gulangu" data-id="97591"><section><section><section data-width="100%"><p><p><img src="image/20201014/4b82fc05ed96107213fa47b41664d758_8.gif" data-autoskip="1"><section><span><strong><span>至于文章提出的三条建议:</span></strong></span></section><section><span>1、A型血患者可能需要特别加强个人防护以减少感染的机会;</span></section><p><span>2、感染SARS-Cov-2的A型血患者可能需要更警惕的监测和积极治疗;</span></p><section><span>3、作为SARS-CoV-2和其他冠状病毒感染的常规管理的一部分,在患者和医务人员中引入ABO血型分析可能会有所帮助,以帮助确定管理选项和评估人们的风险暴露水平。</span></section></section></section></section></section></section><p><br /></p><p><span>可作为进一步研究的临床参考资料,而不能作为现阶段的临床指导和实践,可以为进一步更深层次的研究提供方向。</span></p><section><br /></section><section data-tools="gulangu" data-id="97591"><section><section><section
