解密：瑞典政府对华大基因提出一个奇怪的指控

data-tools="gulangu"><section><section><p><br  /></p><section><br  /></section><section><section><span><span>随着新冠病毒在全球蔓延，对病毒的检测工作变得更加紧迫。在新型冠状病毒的检测上，多数国家用的是华大基因的核酸检测</span>剂。</span></section><section><br  /></section><section><span>早在3月27日，华大集团旗下深圳华大智造科技有限公司（下称“华大智造”）披露，将在瑞典首都斯德哥尔摩共建万人级别新冠检测多组学检测实验室。此外，为确保瑞典万人级新冠多组学检测实验室在3月30日前启动运作，华大智造还派遣技术团队前往实验室一线进行设备安装及培训操作指导。瑞典哥德堡、隆德地区都将派遣技术人员前往支援万人级新冠多组学实验室建设，以实现实验室24小时无休运转。</span></section><section><br  /></section><section><span>然而最近瑞典国家卫生部门最新发布的声明报道，声称由于华大基因提供的PCR试剂盒存在缺陷，导致瑞典国内有约3700例新冠检测结果将被视为无效，这些患者曾被错误地认为染上了新冠病毒。</span></section><section><br  /></section></section><section><section><section><p><img /></p></section><section><br  /></section><section><span>声明指出，华大基因公司提供的这种试剂盒<strong>无法向病毒含量非常低的患者提供有效检测，导致无法正确判定患者究竟是阳性还是阴性。</strong></span></section><section><br  /></section><section><span>瑞典卫生部门官员Karin Tegmark Wisell表示：“因为很多国家在使用这个检测工具，我们非常希望向全球传达有关检测结果不准的信息。”</span></section><section><br  /></section><section><span>Karin Tegmark Wisell补充道，华大基因公司必须针对产品缺陷进行升级，因为这对于新冠病毒检测准确性非常重要。“我们不清楚全球对该检测工具的使用程度，只知道出口量很大。” 她续称，该试剂盒已经被广泛出口到其他国家。</span></section><section><br  /></section><section data-support="96编辑器" data-style-id="22631"><section><section><p><span><strong>华大基因将参与港全民检测项目</strong></span></p></section></section></section><section><br  /></section><section><span>该消息一出，立即被一些西方媒体和“绿营”台媒拿去大肆炒作。</span></section><section><br  /></section><section><span>对于瑞典方面的指控，华大基因也在8月26日给出了一份回应，称公司高度重视，并立刻派出技术专家团队支持，正与客户一同核查问题、解决问题，并就相关情况作出了说明：</span></section><section><br  /></section><section><span>有关新冠病毒检测试剂盒获亚洲、欧美多国等认证，亦经过权威机构和第三方评测，是全球灵敏度最高的试剂之一。对于某些无症状感染者，高灵敏度试剂的核酸检测结果判定为阳性。</span></section><section><br  /></section><section><span>若依据低敏试剂的作为标准，就会被判读为“假阳性”，相反就属于“假阴性”，而“假阴性”对疫情防控危害更大，因此高灵敏度试剂带来的高阳性检出率，针对普通人群为主的疫情防控是有必要。</span></section><section><br  /></section><section><span>这句话用更浅显易懂的方式来阐述就是，新冠病毒的检测试剂存在敏感度的区别，有的试剂对于病毒的敏感度偏高，比如华大基因的这种，能将极少量的病毒都识别出来，得出阳性结果，但也<strong>有的试剂敏感度偏低，会将少量的病毒识别为阴性结果。</strong></span></section><section><br  /></section><section><span><strong><span>倘若</span>瑞典方面使用的是低敏感度试剂，就会将一部分华大基因能识别出的极低病毒含量样本，识别为“阴性”。</strong></span></section><section><br  /></section><section><span>而这，可能才是瑞典方面指控华大基因“无法区分低量的病毒样本和阴性样本，得出假阳性”的由来。</span></section><section><br  /></section><section><span>目前港府即将于9月1日开展的全民检测项目将由三个驻港大陆公司完成，其中就包括华大基因，不过尚不清楚届时华大基因在香港拟采用的试剂盒是否与瑞典属同一批次产品。<br  /></span></section><section><br  /></section><section><span>深圳华大基因在香港注册提供检验服务的机构是“华升诊断中心”，该中心已经于香港的中山纪念公园体育馆内设立了16个火眼气膜实验室。华升诊断中心25日表示，实验室的设备已完成调试，并已获得香港卫生署的EQAP证书，随时可以开始执行普及小区检测计划的检测工作。医护人员在采样中心为自愿参与检测计划的市民采集的样本，会由物流及供应链多元技术研发中心安排运送。香港中山气膜火眼实验室将提供2019冠状病毒核酸检测服务，预估日通量最高可达50万。中心强调参与检测的香港市民DNA和资料均不会被实验室收集，非阳性样本会及时销毁，初步阳性样本将送至卫生防护中心实验室进行验证。</span></section><section><br  /></section><section><span>对于市场存在有关本港检测是否会出现类似瑞典情况的担忧，华大基因表示，在香港成立的华昇诊断中心实验室，在检测出可疑患者，会将这些可疑阳性样本交由卫生署复核检验。若初检结果和复检结果不一致，并不符合“假阴性”或“假阳性”的概念。</span></section><section data-style-id="22631"><section><section><p><br  /></p><p><span><strong>新冠病毒检测的局限</strong></span></p></section></section></section><section><br  /></section><section><span>华大的检测试剂到底有没有问题，我们还并不得知，但肯定的是检测技术的限制是确实存在的。</span></section><section><br  /></section><section><span>在此之前，在美国，CDC自己研制的核酸检测试剂盒，也出现了问题，导致无法有效、迅速地对潜在的感染者进行鉴别，结果让病毒在西雅图社区里不声不响地传播了6个星期。</span></section><section><br  /></section><section><p><img /></p></section><section><br  /></section><section><span>抗体检测的优点，是可以做到快速检测，有的检测甚至可以像使用验孕棒一样，在几十分钟内完成整个过程。但是，抗体检测只是对核酸检测的补充，并不能独立用来作为是否感染的诊断，尤其对于刚被感染尚未产生抗体的患者。而早在今年三月份的时候，华大基因就曾发布公告澄清，<strong>，当前批准上市的新冠肺炎检测试剂主要包括两类：一类是核酸检测试剂，一类是抗体检测试剂，</strong><strong>核酸检测是新冠肺炎的首选检测方法，抗体检测不能作为新冠肺炎确诊和排除的唯一依据，不适用于一般人群的筛查，可作为病毒核酸检测的补充，两者的联合检测被认为会提高检测的灵敏度，减少误诊率或漏诊率。</strong></span></section><section><br  /></section><section><span>不同的核酸检测试剂盒，灵敏度是不一样的，灵敏度过低，也是造成核酸检查出现假阴性的原因。所以一些试剂的误诊率也在所难免，因为情况特殊，之前国家并没有对新冠病毒检测的灵敏度进行过严格的规定。</span></section><section><br  /></section><section><span>再者，核算检测实操其实更麻烦，如果操作不当，就有可能给出假阴性的结果。</span></section><section><br  /></section><section><span>比如，做检测的试剂要放在零下20摄氏度的冰箱里，防止试剂中的蛋白质分解。</span></section><section><br  /></section><section><p><img /></p></section><section><br  /></section><section><span>当然，影响核酸检测结果的还有试剂以及实验操作以外的因素。</span></section><section><br  /></section><section><span>为新加坡政府制造了新冠病毒检测试剂，并且给中国供应了1万份试剂的新加坡国际生物医学研究与开发中心启奥城（BIOPOLIS）的试剂研发团队领导 Sidney Lee 表示，&nbsp;新冠病毒核酸试剂测不准的问题还和病人接受检测的时机以及病毒样本运输条件有关。</span></section><section><br  /></section><section><p><img src="image/20201014/f29e9371036677829e4c5424f6c997e6_6.jpg" /></p></section><section><span>咽拭子</span></section><section><br  /></section><section><span>比如，由于 SARS-CoV-2 主要感染肺部，因此初期患者的咽拭子可能无法截取足够多的病毒。其次，如果病毒脱离人体太久，或运输储存条件不当，那么病毒核酸可能会分解，这也会降低能够检测出来的病毒数量。</span></section><section><br  /></section><section><p><img
